What we handle for pharmacists

A pharmacist’s pay is steady and high from the first year, so the plan is about where it goes: loans, workplace accounts and, for some, a pharmacy of your own.

  1. 01

    PSLF at a nonprofit hospital

Public Service Loan Forgiveness cancels your Direct Loans tax-free after 120 qualifying payments while you work full time, 30 hours a week on average, for a government or 501(c)(3) employer. You must be its W-2 employee; agency shifts there do not count.

  2. 02

    Overtime and shift differentials

The 2025 to 2028 overtime deduction covers only the premium half of overtime the FLSA requires, up to $12,500. Pharmacy is a learned profession, so a salaried pharmacist is usually exempt and gets none of it. A shift differential is not overtime.

  3. 03

    Your 401(k) or 403(b)

The 2026 limit is $24,500, plus $8,000 from age 50 or $11,250 in the years you turn 60 to 63. It is one limit per person across every employer, so a second job’s plan shares it. Catch-ups go in as Roth if your 2025 wages there topped $150,000.

  4. 04

    Your HSA

With a high-deductible plan you can put in $4,400 for self-only coverage or $8,750 for family in 2026, plus $1,000 from 55; employer deposits count toward it. Put in through a payroll cafeteria plan, it skips income tax and Social Security and Medicare tax.

  5. 05

    Backdoor Roth contributions

A direct Roth IRA phases out from $153,000 to $168,000 of 2026 income single, or $242,000 to $252,000 joint. Above that, contribute to a traditional IRA and convert, reported on Form 8606. See backdoor Roth.

  6. 06

    Owning an independent pharmacy

Drugs on the shelf are inventory, deducted as they are sold, not when you buy them. Prescription files, goodwill and a noncompete you buy with a pharmacy are amortized over 15 years. Premier adds the pharmacy’s bookkeeping and return, priced by complexity.

How Nino helps pharmacists

Your pay is predictable, which makes a plan easy to set and easy to neglect. On an Advisor plan your financial planner and CPA set where each paycheck goes, check it every quarter, and file the return that reflects it.

  • A plan in your first two weeks

Your financial planner and CPA build a plan from your pay stubs, benefits elections, loan servicer’s figures and goals, then revisit it every quarter and at open enrollment.

  • Your return, prepared and filed

Your CPA prepares your federal and state returns, including Form 8606 for a backdoor Roth, Form 8889 for your HSA and any overtime deduction you qualify for. Nothing is filed until you approve it.

  • Your accounts in one view

Nino’s AI software connects your workplace plan, HSA, IRAs and loans read-only, so you can ask how far you are from PSLF or what an extra weekend shift adds after tax.

What should pharmacists decide early?

Four choices that steady income makes easy to postpone.

PSLF or a refinance

Refinancing into a private loan can lower the rate but ends PSLF and federal repayment plans for good. At a nonprofit or government employer, compare the forgiveness you would give up with the interest you would save before you refinance.

Roth or pre-tax at work

Pre-tax contributions lower your income now, which can also lower an income-driven loan payment. Roth contributions grow tax-free. A steady salary makes your bracket easy to predict, so the choice is a calculation, not a guess.

Whether to spend or invest the HSA

Paying medical bills from your own cash and leaving the HSA invested lets it grow tax-free; keep the receipts and you can reimburse yourself later. Unlike a 401(k), money spent on qualified medical costs is never taxed.

Whether your pharmacy gets the QBI deduction

Selling drugs is not a health service for the 20% deduction; pharmacist services are. If services bring in under 10% of gross receipts, for a pharmacy with $25 million or less, none of it is. Above $201,750 of 2026 taxable income, or $403,500 joint, the wages it pays start to cap the deduction.
